Please read the following information if you are the parent of a potential American or Intermediate League All Star.

**Verification for All Stars**

A District Representative will be at the field Monday the 29th of April between 5:30 - 7 pm to do All Star verifications. Please read the following information closely and bring the appropriate documentation within the appropriate date range.

What you need to bring:
Player’s Birth Certificate - ORIGINAL MUST BE SEEN BY THE DISTRICT REPRESENTATIVE and a Copy (copy will be retained by the league)

***  IF YOU DON’T HAVE YOUR ORIGINAL BIRTH CERTIFICATE,

YOU WILL BE TURNED AWAY.  ***

Proof of Residency -Three proofs - Must have one proof from each of the three groups!

Must be dated or in effect between Feb 1, 2018– Feb 1, 2019 (No earlier and No later than these dates)

One document from each of the following groups:

GROUP ONE:
1. Driver’s License
2. School records
3. Vehicle records (registration, lease, etc.)
4. Employment records
5. Insurance documents

GROUP TWO:
1. Welfare/child care records
2. Federal records (Federal tax, Social Security, etc.)
3. State records
4. Local (municipal) records
5. Support payment records
6. Homeowner or tenant records
7. Military records

 GROUP THREE:
1. Voters Registration
2. Utility bills (gas, electric, water, phone, waste
3. Financial records (loan, credit, investments, tec.)
4. Medical records
5. Internet, cable, or satellite records

You MUST bring copies of the proof of residency; those documents will be retained by the league.

NOTE: Previous 9/10, Majors, and Intermediate All Stars do NOT need to get verified again this year.
